PLAN DESCRIPTION
Dental Plus of Oregon is a cost saving discount dental plan. By enrolling in Dental Plus of Oregon, you receive a discounted rate for dental services from Willamette Dental Group for yourself as well as your family. As a plan member, you simply schedule your appointments, see the dentist and pay at that visit. When utilizing this plan you will never need to fill out or submit any claim forms. Dental Plus of Oregon makes access to quality dental care easy and affordable.

Membership fees may be paid by credit card (Visa, Mastercard, Discover), check or money order.

HOW TO ENROLL
To enroll in Dental Plus of Oregon, simply complete the application form and submit it along with payment of the membership fees. No physical examinations are required for enrollment and there is no waiting period for access to dental services once you are a member. The application and membership fee payment must be received by the 25th day of the month, preceding the period for which membership is to be effective.
You must be at least 18 years of age and a resident of Oregon to qualify. Your eligible dependents include your spouse or domestic partner and you or your spouse's or domestic partner's children through age 18 or until their 23rd birthday if the child is a full time student.

The Dental Plus of Oregon discount dental plan is not insurance. Plan Members must pay for all dental and ancillary services, but will receive a discount from Willamette Dental Group providers.
TO CHANGE OR CANCEL YOUR MEMBERSHIP
Change requests, such as adding/deleting a dependent, name and address changes, and cancellation requests, must be made writing. To make a change to your Dental Plus of Oregon membership, please submit your request via mail, email or fax as noted below. If you are canceling your membership, your coverage will terminate on the last day of the month following our receipt of your cancellation notice, unless your fees are past due or you specify a later date.

Can I sign up for Dental Plus of Oregon and still go to my own dentist?
To receive the excellent discounts of the Dental Plus of Oregon plan, you must receive care from a Willamette Dental Group dentist or specialist.

How do I schedule an appointment?
To schedule an appointment, please call our Appointment Center toll free at: 1-855-4DENTAL (1-855-433-6825).
The Appointment Center is open:
Monday – Friday from 7am – 6pm PT
Saturday from 7am – 4pm PT
How are membership fee payments made for Dental Plus of Oregon?
Membership fees for Dental Plus of Oregon may be paid annually. You have three payment method options:
Membership fees are due on the 25th day of the month, preceding the period your membership is to be effective. Membership must be renewed annually to continue your membership. Make checks payable and mailed to: Willamette Dental Insurance, Inc., Dental Plus of Oregon, 6950 NE Campus Way, Hillsboro, OR 97124.

Can I drop-off my application and premium payment?
Premium payments are accepted via the methods listed above. Please do not remit payment to your Willamette Dental Group dental office. The dental office cannot accept payments from patients for this discount plan.
How do I know how much Willamette Dental Group charges for dental procedures?

Are there benefit waiting periods under this plan?
No, there are no waiting periods under the Dental Plus of Oregon plan.
