AN INDIVIDUAL
DENTAL PLAN

Dental Plus entitles you to a 25% reduction
in fees from Willamette Dental for:
- General examinations
- Cleanings
- Restorations (fillings)
- Crowns
- Dentures or partial dentures

Members also receive a 15% reduction
in fees from Willamette Dental for:
- Orthodontia
- Oral surgery
- Pediatrics (dentistry for children)
- Periodontics (gum & bone problems)
- Endodontics (root canals)
- Denturist services

Willamette Dental is pleased to offer you Dental Plus, a cost saving dental plan. There is no maximum to the amount of dental services that you can use, nor are there any deductibles that need to be met. This plan gives you simple access to total dental care.

Orthodontia through Willamette Dental Offices is available for both adults and children. When utilizing this plan you will never need to fill out or submit any claim forms. As a plan member, you simply schedule your appointments, see the dentist and pay at that visit. Willamette Dental makes access to quality dental care easy, while Dental Plus saves you money on your dental care.

Participation
To participate in the Dental Plus Plan you must be a resident of Oregon and you must receive your care at a Willamette Dental Office. With more than 60 dental offices through out the states of Oregon, Washington and Idaho, there is most likely an office convenient to you. Advance appointments are required. To schedule your dental or emergency care appointment please call (503) 952-2100 in the Portland Metro Area or toll free at (800) 461-8994.

When you speak to a Willamette Dental representative or arrive at the dental office for your appointment, simply identify yourself as a Dental Plus Member. You will then receive quality dental care at a reduced fee.

Most dental offices are open Monday through Saturday, 7 AM to 6 PM. In making your appointments you can anticipate being seen within the following time-lines: Emergencies; Within Approximately 24 Hours for Pain, Bleeding, or Swelling. First Appointment; 30 Days. Regular Hygiene (Cleanings); 45 Days. Non-Emergency Operative; 60 Days.**

**With the exception of emergencies, the numbers of days are averages. The length of wait-time may vary based on your choice of dentist, location and your desired day or time of appointment.

How To Enroll
To enroll in Dental Plus, simply complete the application and submit it along with payment. No physical examinations are required for enrollment and there is no waiting period for access to dental services once you are a member.

Family memberships may include your legal spouse/domestic partner and unmarried dependent children from birth through age 18 (through age 23 if a registered student in full-time attendance at an accredited educational institution). Dependent children are not eligible for single membership. Membership is effective the first of the month, if you have enrolled by the 25th of the previous month. Dental Plus must be renewed annually to continue membership.

In order to receive reduced fee dental services, charges must be paid in full at the time of the visit. Please check with your Willamette Dental provider for charges on specific treatment. Visa, MasterCard and Discover are accepted. Membership in Dental Plus must be continuous over the treatment period to receive reduced fees.

Plan Exclusions
Plan does not include: Services provided by a dentist; agency; or facility other than a Willamette Dental provider. | Services received before the effective date; or after the termination date of this agreement. | General anesthetic; IV sedation; or hospitalization charges. | Dental treatment unable to be, or not customarily performed in a Willamette Dental Office including cosmetic dentistry. | Dental treatment which your Willamette Dental provider determines to be unnecessary. | Any dental procedure determined by a Willamette Dental provider to be harmful to the member. | Charges by any person other than a licensed dentist; licensed denturist; or licensed hygienist. | Dental Plus is not dental insurance, thus there is no coordination of benefits offered. | Willamette Dental Insurance, Inc. may refuse to renew this agreement only for a reason permitted by law.

Frequently Asked Questions

As a member of Dental Plus of Oregon,
can I go to any dentist I choose?

The benefits of this plan are ONLY offered when services are provided by a dentist or specialist at a Willamette Dental office.

Is this plan offered to Washington residents?

You must be an Oregon resident to be eligible for Dental Plus of Oregon.  However, the benefits of this plan are offered at all Willamette Dental offices in Oregon, Washington, and Idaho.

How do I schedule an appointment?

What can I expect at my first visit?

If you are a new Willamette Dental patient, your dentist will need to examine your current oral health in order to make an informed recommendation for a personalized treatment schedule.  Your first visit will probably include a new patient exam and x-rays.  Cleanings and other preventive procedures can be scheduled after this initial appointment.

What procedures are covered at 25% and
which are covered at 15%?

Any procedure that does NOT require the work of a specialist (specialists include orthodontists, periodontists, endodontists, denturists, cosmetic dentists, pediatric dentists, etc.) is covered at 25%.  This includes any work done by a general dentist or hygienist, and also applies to all lab fees.  Any time an appointment is made with a specialist your discount is 15% for any procedure.

How do I know how much Willamette Dental
charges for dental procedures?

Willamette Dental’s UCR (Usual Customary and Reasonable) Charges are very similar to those you would find at most other dental practices in Oregon.  For questions about the costs of specific procedures please contact our Patient Relations Department at (800) 460-7644 x4393.

How are payments made for Dental Plus Insurance?

Premium for Dental Plus of Oregon is paid annually; prior to the 25th of the month preceding the effective date of coverage.  Premium payments should be mailed to:

Charges for dental services are to be paid in full at the time of service.

Are there any waiting periods under this plan?

There are no waiting periods with Dental Plus of Oregon.  If your payment and application is received by the 25th of the month, then eligibility for Dental Plus of Oregon begins on the 1st of the following month.

Will I receive a Dental Plus insurance card
when I become a member?

Yes.  Your dental Plus ID card(s) will be mailed to you within 10 business days after receipt of premium payment.  However, as long as you are a member of Dental Plus of Oregon, you do not need an ID card to be seen or to make an appointment.
