plan, including our evidence-based treatment approach,
locations, implant services, orthodontia and much more.
If your employer has a customized enrollment packet, please ask him/her to provide you with a benefit
summary/enrollment packet that is provided by a Willamette Dental
service representative.
*This enrollment packet is intended for Willamette Dental plan members that are provided dental

EMPLOYER GROUP ENROLLMENT APPLICATION/CHANGE FORM*
The Willamette Dental employer group enrollment application/change form allows you to enroll
yourself and your family members in the Willamette Dental plan as well as change
information regarding your enrollment.
Once you have completed this form, please submit it to your human resources administrator.
